I have the original novel, and an interesting scene is when Sophie first sees herself cursed, and remarks

"Don't worry, old thing. You look quite healthy. Besides, this is much more like you really are."

After reading on how she behaved after finally leaving the hat shop for the festival, and the book even says: 'Sophie felt as if the past months of sitting and sewing had turned her into an old woman or an invalid.' It seems that she really believes she is like an old woman, and that might be the reason why she doesn't turn back (in the book, that is. She does turn back towards the final battle.) when Howl tries to remove the curse.

That's also exactly what I think as the most feasible and believable answer to the question. In fact it is not (or may not be) the first time that the characters' outward apperances change with their inner state of mind. Examples include Porco Rosso and Bou.

Or maybe Miyazaki wanted to make stuff open for the audience's interpretation (or imagination?), another usual practice in his works.
